The Spaniards spread the Catholic faith to Manila in 1571 when they occupied the city. The zeal of the missionaries made the conversion easy, leading to the establishment of the See of Manila in 1578. The first Cathedral was built in 1600. The present Romanesque structure was dedicated to the Immaculate Conception of the Blessed Virgin Mary, during a time of American occupation of the country.

Pope John Paul II raised the Metropolitan Cathedral to the rank of Minor Basilica in 1981.
